When can I use each application?

What form should I use?
  • MT-EZ
    • Letters of Map Amendment (LOMAs) only
    • Single existing residential structure
    • Not built on fill
    • If the structure was built on fill before 1973 or before the area was identified as a Special Flood Hazard Area on the initial flood hazard map
  • MT-1
    • Letters of Map Amendment (LOMAs), Letters of Map Revision Based on Fill (LOMR-Fs), Conditional Letters of Map Amendment (CLOMAs), and Conditional Letters of Map Revision Based on Fill (CLOMR-Fs)
    • Single or multiple lots or structures
    • Existing or proposed structures
  • MT-2
    • Letters of Map Revision (LOMRs) and Conditional Letters of Map Revision (CLOMRs)
    • If proposed projects, if built as proposed, would meet minimum NFIP standards or proposed hydrology changes (CLOMR)
    • For officially revising the current NFIP map to show changes to floodplains, floodways, or flood elevations.

Supporting Materials

You can find definitions of supporting documentation for the applications here:
  • Plat Map - A map, drawn to scale, that shows a parcel of land and how it is subdivided into individual lots. Plat maps also show the locations of streets and easements.You can obtain a Plat MAP from your local zoning and planning office.
  • Tax Assessor's Map - Tax assessor's maps reflect the legal boundaries and dimensions of each parcel and serve as the basis for land value assessments. These maps show individual tax parcels, road rights of way, subdivision boundaries, and other details.
  • FIRM - Flood Insurance Rate Map -Flood maps produced by FEMA that show Special Flood Hazard Areas, floodways, areas subject to the 0.2-percent-annual-chance (500-year) flood, Base Flood Elevations, watercourses, undeveloped coastal areas, and physical features such as roads and railroads. You can obtain your FIRM from your local Community Map Repository, which usually is located at the Planning and Zoning office, or ordered through the MSC.
  • Elevation Certificate
    • This certificate can be submitted in support of the elevation form, which is required with the application.
    • Used to provide elevation information necessary to ensure compliance with community floodplain management ordinances, to determine the proper insurance premium rate, and to support a request for a Letter of Map Amendment or Letter of Map Revision Based on Fill (LOMR-F) request.
    • Used for structures only (not land).
    • To be completed by a licensed land surveyor, registered professional engineer, or architect who is authorized by law to certify elevation information.

  • Elevation Form
    • This form is included in the MT applications; it is used to certify elevation information for a property. This certification is to be signed and sealed by a licensed land surveyor, registered professional engineer, or architect.
  • Community Acknowledgement Form - Form that must be completed by community official stating that the completed or proposed project meets or is designed to meet all of the community floodplain management requirements.
    • Used for requests involving fill and the regulatory floodway

  • Standard Flood Hazard Determination Form - This certificate is provided by your mortgage company to determine whether flood insurance is required by the Flood Disaster Protection Act of 1973.
